Homemaker
A Helping Hand at Home
Homemaker services assist with general household tasks, including cooking, laundry, errands, and cleaning. This short-term service is available when the usual household member is unable to help. Homemaker services do not include personal care or supervision. This service is available for individuals under 18 living in their family home. For individuals 18 and older, other DHHS services may be more appropriate.
Vocational Services
Empowering Careers, One Step at a Time
Vocational services focus on helping individuals find employment and develop the skills needed to work in the community. These services include prevocational training, supported employment, habilitative workshops, and habilitative community inclusion.

Community Integration: Empowering Independence Through Engagement
Community Integration focuses on teaching self-help, appropriate behaviors, socialization, and adaptive skills. This service mainly takes place in the community, and individuals have control over where and how often they engage with community activities. The service fosters independence, personal choice, and interaction with others in the community. Providers assist with activities of daily living, health maintenance, and supervision.
Family Supported Services
Strengthening Families, Empowering Individuals
Supported Family Living occurs in the individual’s family home. Providers assist in teaching skills for independent living and community involvement. Support includes activities of daily living, health maintenance, social and leisure skills, and community access. This is a habilitative service.
Transportation
Transportation services provide rides to and from waiver services, separate from transportation during services. This service is not intended to replace other transportation options, such as rides from friends or family. It cannot be used for medical appointments, as other DHHS services are available to meet that need.

Provides support to individuals with intellectual disabilities to assist with daily living activities.
Independent Living
Support that empowers your lifestyle and respects your freedom.
Independent Living services are provided in the individual’s own home, where providers help teach skills for living independently and engaging in community activities. The provider offers support with activities of daily living, health maintenance, and social and leisure skills. This is a habilitative service.
Respite Care
Rest for You, Care for Your Loved One
Respite care provides short-term relief for caregivers when they are unable to provide care. This service temporarily supports the usual caregiver who lives with the individual. Providers assist with activities of daily living, health maintenance, and supervision.
Child Day Habilitation
Growing, Learning, Thriving Together
Child Day Habilitation occurs in a community setting, providing support and teaching to meet age-appropriate needs of a child with a disability or special health condition. The focus is on promoting independence, personal growth, and community integration. This service is habilitative, aiming to support the child’s developmental needs
Shared Living
Living Together, Thriving Together
Shared Living takes place in a private home, where a person, couple, or family that is known to the individual provides support. The person providing care, known as a Shared Living Provider (SLP), shares daily life with the individual in their home and is available whenever needed.
Family and Peer Mentoring
Guidance from Those Who Understand
Family and Peer Mentoring offers mentors who share similar experiences with families or participants. These mentors provide guidance, education, and support by sharing strategies and insights to help navigate available resources and systems.
